Insurance:
We accept most local insurance companies.  Please contact our office for specific plans.  We accept non referred and referred patients.  Please check with your insurance if a referral is needed.

Payment:
Co-payments and deductibles are due at the time of service.  We accept cash, check, and credit cards.  All other claims will be filled with your insurance.

Cancellations:
Please cancel appointments/procedures 24 hours in advance so other patients may be scheduled in your place.  Failure to cancel appointments/procedures 24 hours in advance will result in a missed appointment fee($25) or procedure fee($100).

Medication Refills:
You may contact our office during regular business hours to request medication refills.  The request will be verified by our physician or physician assistant.  Please allow at least 24 hours for refills to be called in.  Late calls on Fridays may not be called in until after the weekend.

Test Results:
You will be made a follow up appointment to go over test results that are ordered for you.  You may contact us for results during regular business hours.  Please allow 7-10 days after your test is performed for us to get results back before calling.
